Pilot teams and others such as the Manhattan ACTT team of Visiting Nurses Services note the need for advanced planning and dialogue to support "fit" of medication with the needs of the client in their recovery process.
Manhattan ACTT has developed a Medication Tier assessment that fits well the recovery model and even matches the dual perspective approach of worker/client we use in the Ontario Common Assessment of Need (OCAN).
The medication tier assessment (first file uploaded below) is matched with a Recovery Assessment (2nd file). These were provided to me by MacDara O'Sullivan. He notes that this is some kind of Beta model that has not been validated as a clinical practice tool. For me, it is promising as a resource for ACTT.
