Yes Season, the actual practice is where it is at.  The role of leadership is often quoted in implementation and Knowledge Translation and while for sure senior management role is vital, it seems pretty trickle down. 

This journal article flushes the topic of system/organizational change quite well - a bit refreshing in it's honesty.

I feel for front liners to enter practice change, you have bring it to the busy clinical team - ie: everyday  Kardex and make relevant to what needs to be done...today.
