An update to the investigation into Ashley Smiths death in corrections in 2007. After the coroners inquest the jury tabled 70 recommendations to Corrections Canada, Howard Sapers, Canada's Correctional Investigator pushed for adopting these well thought out recommendation, however Corrections Canada will NOT be adopting any of these changes. Sad day.
