The Summit Housing and Outreach Programs ACT Team recently adopted the ATR scale.  As SHOP is trying to reduce paper records and keep client records in CRMS, it made sense for us to have a fillable template in CRMS for the ATR scale.  

For each question, there is a drop down menu of the rating statements and when completed, the score is automatically tallied for you.  We are making this template available at no cost to any agency using the ATR and CRMS. If your agency is interested in obtaining the fillable ATR form for CRMS please let me know and I will give CRMS permission to upload it to your CRMS database.  You will have to provide me with the name of the person who would have authority to approve the form being uploaded so that CRMS can verify it's ok to do this.  I can be contacted at Irene Zivko izivko@summit-housing.ca

Hi all, just checking in Mukib, how it is going for you and your team on the case management software front, both:

  • if CRMS form is flying/working
  • and how team members and your program is getting feedback/reports on their efforts?

Yesterday, had a useful discussion with Summit's -Irene B. and Caroline on management of the team reports - on scores over time, by using the raw data from crms report and posting into EXCEL.  It actually is a fast way to organize client's scores over time, but more to the point... take further steps in recovery/treatment plans for the individual clients, in team discussions.

As usual, I continue to encourage teams to link the 7 ATR themes in the 18 brief questions - (ATR guided by the LOCUS dimensions)  http://66.63.77.54/affiliate/LOCUS.pdf to their use of the OCAN in the ongoing effort to have our assessment efforts bridged to everyday work of clients and teams.

Here is the working sheet that helps teams discuss/plan care. (the actual Word document in the Resources section - right side of this page), by summarizing in a few phrases on the right side of the sheet - how going for client in each of the 7 ATR themes.
