From your experience helping ACTT Clients who have been with your team, do you find that current physical health issues can eventually become the primary need of the client, rather than mental health needs?

Are the six framework principles broken down into actions for results (starting at page 12) of the report MENTAL HEALTH AND PRIMARY CARE POLICY FRAMEWORK found in this post https://www.eenetconnect.ca/top...y-framework-released&nbsp relevant, for ACTT teams to work with primary care providers, to ensure effective care for the people transitioned from ACTT?  

What client supports are necessary for both access and maintenance of care are missing from the perspective of ACT Team clients?

 will be useful to bridge with MENTAL HEALTH AND PRIMARY CARE POLICY FRAMEWORK.

...It will translate science into practice so the association's members can continuously improve what they do. It can also shape the overall evolution of how primary health care is delivered so that it meets the needs of individuals and populations."
