These three one hour webnair's by the Recovery to Practice group http://www.samhsa.gov/recovery-to-practice
have focus on bridging the ideas/values of Recovery to actual-- on the ground, practical implementation, in the context of multiple organizations and groups within "a community."  
 
As well, of note is that RTP  as part of  SAMHSA , developed within various professional groups, like nursing, psychology, peer support, psychiatry, addictions... recovery focused approaches calibrated to their scope of practice.

Recovery happens where people live, work, worship and play. Involvement in communities can help people find a sense of belonging, provide access to activities and resources, and offer opportunities for contribution to others. All too often, however, community involvement – in work, social activities, religious life, and other pursuits – is considered only after treatment is completed, rather than being a source of discovery and healing in itself.  

This series will examine how recovery-oriented practitioners and organizations can identify, access, and partner with everyday community resources to help people expand their connections in communities of their choice.

I have found this resource below useful, as a front line worker who is working within various systems/organizations to help clients.  

It provides structure for self and perhaps organizational  reflection on the skills, values  and approach needed; a bit of an island, when the old system's got you down, (not the band , System of a Down https://youtu.be/CSvFpBOe8eY?t=3

The British Government back in late 2000's focused on integrating and supporting the care workforce to meeting social inclusion efforts through their National Social Inclusion Programme and National Workforce Programme.
