The Upstream Lab at the MAP Centre for Urban Health Solutions at St. Michael's Hospital is developing the SPARK Tool to collect data on people’s characteristics (such as gender, language preference, race) and social needs (such as income, housing, food security) in health care in Canada! Social needs can impact a person's health. Understanding someone’s characteristics and social needs can help improve healthcare.

To develop and validate the SPARK tool, they need a large number of Canadians to complete the tool so that they can make it better.

They are inviting you to help complete the SPARK Tool.

This research survey is part of the SPARK project led by Dr. Andrew Pinto from the Upstream Lab at the MAP Centre for Urban Health Solutions at St. Michael’s Hospital.

They will ask you to complete the SPARK Tool which has questions about your age, postal code, language preference if you were born in Canada, race and ethnicity, disability, sex at birth, gender identity, sexual orientation, education, ability to meet basic needs, access to food, medication, transportation, utilities and the internet, housing status, access to social or family supports and employment status. They will also ask you to complete a similar tool for comparison to the SPARK Tool.

The survey will take about 20 minutes to complete. Your participation in this study is completely voluntary; you can stop the survey at any point. Your decision to complete this survey or not will not impact the care you receive at Unity Health Toronto now or in the future. You must be 18 years or older to participate in this survey.
