The Centre for Addiction and Mental Health, Jean Tweed Centre, Centre for Innovation in Peer Support, and Addictions Mental Health Ontario would like to hear from you! 

Let us know what you think by completing this anonymous survey regarding digital health resources during COVID-19 and beyond.

This survey is being broadly distributed across Ontario, to explore current needs and priorities in this area, and to inform the development of services and supports for those who identify as female or women, and who struggle with substance use concerns.

The questionnaire is completely voluntary and anonymous, and all responses will be kept confidential. You may skip any questions you do not want to answer and quit the survey at any time. The survey includes 7 items and will take approximately 5-10 minutes to complete.
