To celebrate the Change Foundation's 25th Anniversary and to showcase the work of our 2017-2020 strategic focus on family caregivers, we invite you to our Capstone Summit in May.
The Change Foundation and our Changing CARE partners will share learnings, insights and details about our work with caregivers in the healthcare system to better recognize, support and integrate family caregivers in health and community care organizations.
Over the two days in May expect:
Practical 'how-to' sessions by the Changing CARE teams on using co-design and engagement to improve quality of care.
Honest conversations with caregivers about how their experiences changed with the introduction of caregiver-friendly initiatives.
An overview of co-designed resources and tools available to improve the caregiver experience within any organization, partnership or community.

In honour of our 25th anniversary, the Foundation wants to recognize innovators and put a spotlight on people, organizations and partnerships that are trail blazers and will likely make important and meaningful change in the next 25 years.
To do this, we launched the Future Innovator Awards. The Future Innovator Awards will recognize up to five people, teams and/or organizations that have a strong vision for positive change for patients and caregivers and have the potential to make notable impact going forward. The nominees should have some track record of driving change, but most importantly have the potential to create and lead transformative ideas in the future.
An awards ceremony will take place in the evening on May 4th. Details to follow.
