Families and individuals caring for those who are transitioning from youth to adult health care may face many new challenges. In partnership with Ontario Health, this webinar will inform caregivers on the importance of coordinated transitions and resources that can help support them and their care recipients during this experience. You will hear from three caregivers who have cared for young people with different health conditions and needs and who have experienced the transition from youth to adult health care services first hand.
Webinar participants will learn how to:
• Understand the importance of coordinated transitions between youth and adult health care services to prevent negative health outcomes and ensure continued health system use
• Access the Transitions From Youth to Adult Health Care Services Quality Standard and the Guide for Caregivers on the Health Quality Ontario website
• Identify tools and highlight resources to help caregivers through the transition process
• Recognize areas for improvement to enhance the transition process for themselves and their care recipient
