Enabling Seamless Communication Between Partner Agencies

I am interested in learning from others who may have set up interfaces between partner EMRs to enable the communication and transfer of information between partner agencies.
What was your experience? Any lessons learned?

Good question, Carly. The Dacima system that we use in YWHO that [@mention:337383271028434279] is referring to, is shared among partner agency staff involved in the youth's care. The system does not currently operate as an EMR, however. I think that you are looking specifically for EMR to EMR interfaces that allow for this transition?
I wonder - do any of the Youth Wellness Hubs Ontario service providers know about an EMR to EMR transfer interface?

This other paper specifically addresses Electronic Health Record,EHR, quite useful as rigourous and gets into the everyday use - frontline process of EHR.
I think it waves away much of the confusion around EHR in the context you are aiming of using it.
